...the first time he chooses the Traveller. Is this the one and only time that this happens?
We ruled ''Yes, the reader makes a second guiding question only at the beginning of the game when he/she chooses a traveller ''.
Were we correct? And if yes, why is this rule like so?

I've played this several times but we've stopped using the terms "reader" and "traveler" so I'm not sure what you're asking. Can you rephrase with an example?

The reader is the player who reads the book and decides who will be the first player (traveller) that will start the game by giving him his penny. The traveller then draws a memory trigger (the paper with a phrase written on it) and reads it to the other players (guides). The guides then start making one guiding question each, except the reader that gets to make a second one. After this phase the traveller says ''I remember a time when...'' and then narrates his memories according to the guiding questions.

I don't think I can authoritatively answer your question, but since RPG Geek is still a relatively small community, I'll do my best rather than hope someone else comes by and answers it.
When we play, the Reader gives his penny to a player of his choosing, and from then on the Reader is just another player. When we play the Reader does not get to ask a second triggering question. I'm not sure why he would, and I'm surprised to hear that the rules indicate that he should.

A Penny For My Thoughts, p.19 wrote: The Reader will ask an additional Guiding Question of the first Traveler. For subsequent Travelers, any guide who has given a penny to that patient during the Choose the Traveler phase as per What If No One Has Enough Pennies to Be the Next Traveler?, p 15) will ask an additional Guiding Question.
So getting to ask a second question is linked to having to give a penny in order to break a stalemate in the process. I'm not sure *why* this is necessary, though.

Guess we missed that rule... Maybe it's supposed to be a sort of compensation for starting with one fewer penny?
We don't play with that rule and the game is great the way we play it, so it can't be that important.
