We had a question regarding how the Fight/Flight mechanics work for DRYH come up during our game here in the play-by-post forums, and it's been a point of confusion for some of my real life players as well.
I figured I'd post my response here in case anyone else has the whole "what really happens when madness dominates" question come up during a game.
The long-winded answer
At character creation you have 3 total responses that you can assign to either fight or flight. These values represent the MAXIMUM number of times you can "check off" either of these responses. The value is "reset" back to empty whenever you snap, or you can reduce them when Discipline dominates.
Think of these choices as a limit gauge in a final fantasy game or a special gauge in any fighting game, except that when these gauges are both full, instead of doing something incredibly cool, you just go batshit crazy (...and possibly do something incredibly cool).
These values can never exceed what you chose at the beginning of the game.
Let's say we were playing a character that chose to go with 1 Fight and 2 Flight.
Here are two examples of how this could look on your character sheet:
If you choose to use Fight, you'll "check off" (mark) your single fight box
This means that you can't use the fight response again (at least not until you snap or you choose to reduce it after Discipline dominates).
The next time madness dominates and you have to select a response, you'll be forced to choose "Flight".
Once all of the boxes have been checked, you'll reset the check boxes and take a point of permanent madness, reducing your permanent discipline by one.
You just made a roll with madness dominating! 1.) "Oh no! I can't check off another box!" 2.) "Okay, I'll put my responses all back to zero"
3.) "Then, I'll increase my permanent madness by one and decrease my permanent Discipline by one."
After you've snapped at least once, you'll be rolling two discipline and one madness instead of your normal three discipline. If you were to snap again from filling all of your responses, you'd roll one discipline and two madness. Snap again? Uh oh, bad news. You're now haunting the streets of the city that sleeps.
Hope that makes more sense! Let me know if you have any other questions.

This makes me really want to play in a massively-multiplayer-by-forum (MMOPbF), in a west marches style, with tons of individual 1-on-1 games of Don't Rest Your Head in the same city.

The game really is a ton of fun, especially when you reach the crossroads between two different character's stories.
I would personally LOVE a large number of one-on-one sessions with this game.
The long-term effects of individual characters on the world could be a ton of fun too. Kind of like Living Forgotten Realms, except with actual role-playing and long term consequences.
In fact...I'll go so far as to say, if anyone EVER wants to do a one-on-one game of DRYH with the chance possibility of their story running in with another player, send me a tell, or post in the OOC thread and we'll get you started.

Super explanation David and I am relieved that your real life players got also confused
By the way it's such an awesome game and David is setting the tone so wonderfully I often read his posts and get genuinely shocked.
